What is a medicated shampoo?
A medicated shampoo contains an active ingredient intended to manage a particular scalp condition or symptom. Depending on the product, that ingredient may help control yeast, loosen scale, reduce build-up or manage the excessive skin-cell production associated with psoriasis.
Medicated shampoos are not interchangeable. Ketoconazole may be used for fungal or yeast-related scalp conditions, while salicylic acid may be used to soften and loosen thick scale. Some psoriasis shampoos contain coal tar. The right choice depends on what is causing the problem, the severity of your symptoms and any other treatment you are using.
Always follow the label. Do not leave a medicated shampoo on longer, use it more frequently or combine it with other active treatments unless the instructions or a healthcare professional advise you to do so.
When might an itchy scalp need medicated shampoo?
Itching alone does not identify the cause. Look at the wider pattern of symptoms and consider how long they have lasted.
Dandruff or seborrhoeic dermatitis
Dandruff commonly causes loose flakes and itching. Seborrhoeic dermatitis may also cause redness, irritation and white or yellowish scale, and it can affect the eyebrows, sides of the nose, ears or beard as well as the scalp.

Scalp psoriasis
Scalp psoriasis often produces thicker, more clearly defined plaques with silvery scale. The scalp may also feel itchy, sore or tight. Medicated shampoos containing ingredients such as salicylic acid or coal tar may be included in a psoriasis care plan, but shampoo alone may not be enough.

A scalp condition that has not improved with gentle care
If you have already removed obvious irritants, simplified your routine and used gentle cleansing consistently but the itching and flaking continue, it may be time to ask a pharmacist or GP whether a medicated option is appropriate.
Do not assume that stronger shampoo is automatically better. Persistent itching can have several causes, including eczema, psoriasis, seborrhoeic dermatitis, contact irritation, fungal infection and head lice. An accurate diagnosis helps you avoid repeatedly using products that do not match the problem.
When might gentle shampoo be enough?
A medicated shampoo may not be needed when the scalp is mildly dry, tight or sensitive without persistent scale, marked redness, sores or spreading symptoms. Irritation can sometimes follow a change in hair product, frequent washing, very hot water or the use of strongly fragranced products.

Often, yes. They do different jobs. A medicated shampoo may be used on particular days according to its instructions, while a gentle shampoo may be used when additional washing is needed. However, the routine should be guided by the specific medicated product and your scalp condition.
If you are using prescribed scalp treatment, ask your pharmacist, GP or dermatologist how shampoo should fit around it. Do not stop or alter prescribed treatment because your scalp temporarily feels better.
